Mesothelioma is type of malignancy that is most frequently associated with prolonged exposure to asbestos, accounting for almost 80% of all mesothelioma cases. Symptoms of this condition usually occur after 15 to 20 years of exposure. Miners, shipbuilders, pipefitters and manufacturers of tiles and cements who get to inhale asbestos are at increased risk of developing malignancy later in life.
Mesothelioma is characterized by the involvement of the mesothelium or the lining of the chest and abdominal cavities. A thin membrane called pleura and peritoneum lines these cavities. Though most cases, approximately 50%, of this condition involve the pleura and the peritoneum, some cases involve the lining of the heart called the pericardium, as well as that of the testes called the tunica vaginalis.
The most common presenting signs and symptoms of mesothelioma are shortness of breath and chest pain. When subjected to radiology or other imaging studies, pleural effusion is observed. This is a condition characterized by the presence of significant amount of fluids in the pleural cavity, which can occur bilaterally.
Other clinical manifestations of this disease include wheezing, a high-pitched musical sound heard during expiration and is an indication of airway obstruction and easy fatigability usually caused by inadequate entry and delivery of oxygen to the tissues. Another clinical manifestation is hemoptysis or expectoration of bloody sputum, usually caused by a direct damage to the parenchyma of the lung.
How does it develop? Asbestos fibers, when inhaled, lodge to the parenchyma and pleural lining of the lung. This may produce an inflammatory response in an attempt to get rid of the foreign substance inside the body. The inflammatory cells involved in the process generate reactive oxygen species that aim to "kill" the offending microbe.
The generation of these oxygen species in excessive amounts can lead to the production of free radicals that can cause mutation in cellular DNA. Chronic inflammatory response subsequently leads to the development of a malignant tumor.
There are several ways to diagnose mesothelioma. From the history of the patient alone, you can already tell the possibility of asbestosis or malignancy. The patient's occupation history, not to mention his presenting signs and symptoms, already tells a lot about the diagnosis. It is rarely associated with smoking. However, smoking can accelerate the process of degeneration to malignancy.
A person presenting with difficulty of breathing typically undergoes chest X-ray, an imaging procedure most commonly used as an initial diagnostic tool. As mentioned above, fluids can be seen on imaging. This may lead the doctor to think that the patient has pneumonia and prescribe him with some antibiotics. Repeat X-Ray after antibiotic therapy may reveal clear lungs.
What leads a doctor to consider malignancy is the recurrence of symptoms. When the symptoms and pleural effusion recur, the physician or the surgeon is then prompted to perform biopsy via Video-assisted thorascopic surgery or VATS, which will then reveal mesothelioma.
Once diagnosed, the patient starts on appropriate treatment. Chemotherapy is the only mode of treatment that can improve survival among patients with this condition. It is also possible to perform radiotherapy after the surgery.
