Rosacea is a chronic skin condition affecting the skin of the face and less commonly, the eyes. Ordinary symptoms are redness, acne-like bumps and blemishes, and patches across the skin of the face. Rosacea is a very common, yet a very complex skin condition and because of this complexity it is classified into four different subtypes. A person may be affected by one or more subtypes and treatment differs according to the severity and complexity of the symptoms.
Subtype 1: Erythematoteleangiectatic rosacea
This subtype is characterized by facial redness occuring consistently. Spider-like blood vessels may also be seen. This type of rosacea is typically harmless but may also cause minimal facial discomfort. Treatment for rosacea of this subtype may be challenging as it is not usually treated with medications and pharmacological agents because of its mild nature. Avoidance of individual and environmental factors that trigger the exacerbation of rosacea is important. It is advisable to keep a diary of the events that lead to flare-ups so that you may avoid these events in the future and keep the symptoms from worsening . The reddish appearance of the skin may also be concealed by various cosmetics and skin care products available in the market today. Products that are fragrance free and dermatologically tested for sensitive skin are recommended to be used.
Subtype 2: Papulopustular rosacea.
In this subtype, permanent redness of the face occurs together with acne-like pimples and bumps that are called pustules and papules respectively. This subtype is commonly confused with acne and occurs after or together with subtype 1. Treatment for rosacea of this type includes antibiotics that are taken orally , and topical agents that may be used alone or together with an oral antibiotic therapy. Tetracycline antibiotics are most effective for this subtype specially when used with topical therapy. Metronidazole is the topical agent of choice. Extensive studies have proven that with the right amount of dosage, these pharmacological agents are effective even for long term use.
Subtype 3: Phymatous Rosacea
This is characterized by thickening of the skin, occurance of nodules, and enlargement of bumps or papules. The most commonly affected area is the area around the nose. This subtype commonly affects the male population. Treatment for rosacea of this subtype depends on the severity of the case. Mild cases are resolved by consistent treatment of oral and topical medications, but moderate to advanced cases may require surgery. There are various types of surgical procedure that may resolve this skin condition such as cryosurgery, electrocautery and laser surgery with dermabrasion. All of these surgical procedures reduce the thickening of the skin, reshapes the nose and improve the skin's overall appearance.
Subtype 4: Ocular Rosacea
In this subtype, there is affectation of the eyes and various eye symptoms occur such as dryness, itching, burning, stinging and blurred vision. Eyes may also appear blood-shot and watery. Treatment for rosacea of this subtype also depends on the severity of the cases and whether or not it occurs with different subtypes. Mild cases of ocular rosacea are treated with oral antibiotics and application of artificial tears to prevent dryness and itching. Moderate to advanced cases may require a visit to an opthalmologist for proper prescription of ophthalmic medications. It is very important to give attention to proper cleansing of the eyes and eyelashes with water and clean washcloth. Proper handwashing is also important to prevent harmful microorganisms from entering the mucous membranes of the eyes. An untreated ocular rosacea may lead to further complications and a decrease in visual acuity.
