In vitro fertilization or IVF clinics specialize in infertility problems that cannot be treated with non-invasive treatments, hormonal medications, and other conventional treatments. Some of the procedures that IVF clinics provide are briefly discussed below.
Assisted conception techniques are the more specialized procedures for treating complex infertility problems. Assisted conception control the way that the sperm fertilizes the egg. Treatments classified under assisted conception consist of three main kinds. These are intra-uterine insemination or IUI, in vitro fertilization or IVF, and intracytoplasmic sperm injection or ICSI. All three treatments can be performed in IVF clinics.
Intra-uterine insemination (IUI) places fast-moving sperm inside the female's womb as close as possible to the ovulation time. This is the primary method prescribed to couples when the usual treatments are ineffective. It can be effective for females with mild endometriosis, a disease where in the womb's lining grows where it should not be, e.g. on the ovaries, behind the uterus, or on the bladder or intestines. This procedure is also useful for males who have problems with their ejaculation, or else have mild abnormalities in sperm quality. The procedure can be complementary to ovulation induction.
IVF, or in vitro fertilization, involves removing the eggs and mixing them with the sperm outside the female's womb. IVF is the treatment of choice in cases where there is unexplained infertility, when the female's fallopian tube is blocked, and when IUI and medication have not been effective.
Upon mixing and fertilization has taken place, the zygote or fertilized egg is placed back inside the womb where further development happens. IVF can be performed either with the original couple's eggs and sperm or with donor eggs and/or sperm in cases of abnormality in the former. In addition, embryos and sperm can be frozen for use in the future. This involves some very strict legal conditions and is highly regulated. The specialist will determine if this is warranted, in consultation with the couple.
IVF is undertaken in conjunction with medications too. These medicines control the timing of ovulation so that the period of harvesting of the mature eggs can be accurately predicted. Although one monthly cycle only produces one egg in humans, superovulation can be induced, wherein several mature eggs are produced simultaneously. This is done through the intake of three hormones administered at various specific periods. Superovulation will increase the chances that the female will become pregnant.
The eggs and sperm are collected at the same period, processed and carefully prepared, and mixed together in a laboratory setting. Success is determined in as little as sixteen to twenty hours, and up to two embryos are transferred to the womb if the female is below forty years old. For older women, up to three embryos are transferred.
The third method is ICSI or intracytoplasmic sperm injection. This procedure is essentially the same as IVF, except that in the lab, only a single sperm is injected into the egg, with the resulting embryo being transferred to the female's womb. ICSI is warranted when the sperm has serious abnormalities. This is also done in cases of vasectomized males and when there are fertilization problems between the sperm and egg.
