In a community-based survey among males between 40 to 70 years old dubbed as the Massachusetts Male Aging Study, 52% of the men had some degree of erectile dysfunction. According to the research, the major risk factors for developing sexual dysfunction in males are diabetes mellitus, hypertension, heart conditions and high cholesterol. There is also a higher incidence of erectile dysfunction in men who have gone through radiation or prostate surgery as therapy for prostate cancer.
There is a modern classification of ED recommended by the International Impotence Research organization that groups ED based on the cause: psychogenic, organic or mixed organic/psychogenic.
- Psychogenic – this refers to pure psychological causes of male erectile dysfunction which could be attributed to a general unresponsiveness to sex because of a lack of arousability. This lack of sexual stamina may be primary or can be due to an age-related decline in libido. The psychogenic erectile dysfunction may also be of the situational type which can be partner-related, due to anxiety or fear of failure in sexual performance or can be mood-related.
- Organic – the organic type of erectile dysfunction may be due to dysfunction of the brain, spinal nerves or autonomic nerves that can prevent normal erection and sexual functions. Other organic cause includes hormonal problems such as hyperthyroidism, hypothyroidism and Cushing' s disease that are all known to cause decrease in libido. Arterial disorders and abnormalities in the cavernosal tissues of the penis can also cause erectile dysfunction.
- Mixed organic/psychogenic – this is the most common type of ED which is caused by a combination of both psychological and organic factors.
Treatment of erectile dysfunction has changed radically over the years. In the past, surgical correction through prosthetic surgery was the treatment of choice but modern research and technology has offered less invasive, non-surgical options for men with impotence. There are specific and non-specific options for affected individuals. The former include lifestyle changes such as diet, exercise and smoking cessation to prevent atherosclerotic changes and further damage to the penile blood vessels. Psychotherapy is also recommended for those with a psychogenic cause of erectile dysfunction to identify and get rid of the underlying cause such as anxiety and partner conflicts. Specific treatments for ED include Sildenafil (Viagra) and the new drug Tadalafil (Cialis) that acts similarly in enhancing penile erection by inhibiting the substance that signals the brain to end an erection. Viagra is effective for patients with organic as ell as psychogenic and mixed-etiology erectile dysfunction.
